If the employee sought medical attention for a workplace injury, or is absent the day after the work-related accident, the employee and his/her team leader/manager are required to complete the appropriate Workers’ Compensation Forms, depending on the employee’s province of work.
The manager must also complete a Hazardous Occurrence Investigation Report, in accordance with the OHS Reporting Requirements.

Atlantic
New Brunswick
The employee and team leader/manager must complete Form 67, indicating Employer #75254 and Operation #254.
Newfoundland and Labrador
The employee must complete Form 6 (PDF, 71 KB).
The team leader/manager must complete Form 7 (PDF, 479 KB) indicating Firm #2018821.
Nova Scotia
The employee and team leader/manager must complete the Injury Report (PDF, 1.4 MB), indicating Business #100-254.
Prince Edward Island
The employee must complete Form 6 (PDF, 5 MB).
The team leader/manager must complete Form 7 (PDF, 3 MB) indicating Operation #30.
Quebec
The employee must complete Form 1939 (PDF, 359 KB) and have their physician complete the CSST 1936* Medical Attestation form.

Employee working in Quebec
The employee must complete Form 1939 (PDF, 359 KB) and have their physician complete the CSST 1936* Medical Attestation form.
