If the employee sought medical attention for a workplace injury, or is absent the day after the work-related accident, the employee and his/her team leader/manager are required to complete the appropriate Workers’ Compensation Forms, depending on the employee’s province of work.

The manager must also complete a Hazardous Occurrence Investigation Report, in accordance with the OHS Reporting Requirements.

• Atlantic

  • New Brunswick

    The employee and team leader/manager must complete Form 67, indicating Employer #75254 and Operation #254.

  • Newfoundland and Labrador

    The employee must complete Form 6 (PDF, 71 KB).

    The team leader/manager must complete Form 7 (PDF, 479 KB) indicating Firm #2018821.

  • Nova Scotia

    The employee and team leader/manager must complete the Injury Report (PDF, 1.4 MB), indicating Business #100-254.

  • Prince Edward Island

    The employee must complete Form 6 (PDF, 5 MB).

    The team leader/manager must complete Form 7 (PDF, 3 MB) indicating Operation #30.

• Quebec

  The employee must complete Form 1939 (PDF, 359 KB) and have their physician complete the CSST 1936* Medical Attestation form.

  The team leader/manager must complete Form 1940 (PDF, 658 KB)* indicating Experience File # 38800002 and ETA # 600888433.

  • Employee working in Quebec

    The employee must complete Form 1939 (PDF, 359 KB) and have their physician complete the CSST 1936* Medical Attestation form.

    The team leader/manager must complete Form 1940 (PDF, 658 KB)* indicating experience file # 38800002 and ETA # 600888433.

  • Employee working in Ontario

    The employee must complete Form 6 (PDF, 223 KB).

    The team leader/manager must complete Form 7 (PDF, 421 KB) indicating firm #810942.

    The employee must complete Form W1.
