OHS Reporting Requirements

Module Vl: These occupational health and safety reporting requirements are identified by the Canada Labour Code - Part II, the Canada Occupational Health and Safety Regulations (COHSR) and the Department.

1. Reports To Be Completed By Work Sites

1.1. Hazardous Occurrence Investigation Report (LAB1070) (opens new window)

The employer shall, without delay, investigate, record and take action to correct hazardous conditions that led or could lead to any work-related accident, incident or illness.

This work must be done with the participation of the Workplace Health and Safety Committee/Representative using the LAB1070 form to record and report the results of the investigation and the corrective action taken regarding the hazardous occurrence. It is important to ensure that all signatures are included on the report.

  • In addition, management must consult the ROHSA in the event of any of the following hazardous occurrences, and report them to the Labour Program Office within the applicable time frame:

  • Death of an employee
  • Disabling injury to 2 or more employees
  • Loss of a body member or complete loss of usefulness of a body member
  • Permanent impairment of a body function
  • Explosion
  • Boiler or pressure vessel fire or rupture
  • Unserviceable or free-falling elevator

In addition to the 24 hour report to the Labour Program, the employer must send a written report (within 72 hours) to the Workplace Health and Safety Committee or Health and Safety Representative in the following situations:

  • Boiler or pressure vessel fire or rupture
  • Unserviceable or free-falling elevator
  • Disabling injury
  • Electric shock, toxic atmosphere or oxygen deficient atmosphere that caused lack of consciousness
  • Rescue, revival or other similar emergency procedure. (Emergency Procedure includes the summoning of an ambulance for an employee, regardless of the medical cause.)
  • Fire or explosion

  • Completed copies of LAB1070 must be retained by the employer for a period of 10 years.

This report must be completed by all workplace health and safety committees (WHSCs) based on its activities conducted in the previous calendar year.

  • The employer co-chairs of the WHSC must submit signed copies of the report to the Management/Site Lead and to the ROHSA by February 1.
  • Copies must also be posted in the work site for two months.

2.1. Employer’s Annual Hazardous Occurrence Report (LAB1009) (opens new window)

  • This report is completed each calendar year by the ROHSA on behalf of all local work sites in their region using information from the LAB1058 and LAB1070 reports.
  • The ROHSA will provide this report to the National OHS Office by February 15 of each year.
  • Reports will be retained by the National OHS Office for a period of 10 years.
